Accepting New Patients

We also believe that enjoying what we do – and creating a positive atmosphere for both our patients and team – helps make every visit a pleasant one.

We strive to keep appointments running efficiently so that your time with us is smooth and stress-free. Staying on schedule allows us to give each patient the attention they deserve while keeping visits as prompt and effective as possible. Your time matters to us, and we work hard to ensure every appointment is productive and every experience is positive

Cancellation Policy

When an appointment is scheduled, our staff sets aside time to spend with you individually to hear your concerns and discuss the best solution and treatment for you. If you are unable to keep this scheduled appointment, we require a 24-hour notice. Our cancellation policy in this office is strictly followed due to the increase in our patient volume and the lack of dental care in our area. If you or your family have three no call, no show appointments unfortunately you will be dismissed from our practice. Thank you for understanding.

New Patients

We recommend that new patients arrive 10-15 minutes before their scheduled appointment to complete the necessary paperwork for their first visit. Filling out these forms accurately and completely helps us provide the safest and most personalized care possible.

You can find our new patient forms and consent form on our website. Feel free to print and complete them in advance to make your first appointment go more smoothly.

The patient information forms include details such as:

• Your contact information and medical history
• A list of any medications you are currently taking
• Any surgeries or medical conditions you’ve had

On the back of the form, you’ll find our Financial Responsibility and HIPAA Disclosure sections. If you are unable to complete the forms ahead of time, we’ll provide them when you arrive.

Please also bring your dental insurance card with you to your first appointment so we can scan and verify your coverage. Although our office accepts insurance on an Out of Network basis, we will still gladly submit your insurance claims on your behalf.
